Seek professional help, counselling, medication if needed. If mainstream medication is not possible, go for alternative systems.

You need to get to a doctor of some sort. If you can't afford a specialist, a medical doctor will do. Medicine works wonders; it just takes a couple of weeks to kick in. Once you start feeling better, you will start wanting to do other things, like exercising or socializing with friends. Don't give up. There is help out there.

You need to build your self-esteem. It is going to take a lot of work and time. So it would be wise to be patient on this. You can't expect to change overnight and get discouraged.
If you are suicidal, you need to seek help. Especially if you have a plan. Call a suicide hotline; they are very helpful. Talk to someone you trust. Let someone know about your feelings and intent. You have to help yourself. You cannot let yourself down.
Many people feel the same way you do, and struggle with these matters. And many of them have overcome it. So there is hope.
How exactly do you overcome these problems? Each person's struggle is unique. If you don't want or can't seek professional help, then I'd recommend that you read a few self-help books on anxiety and self-esteem. Maybe join a support group online. There are several on Yahoo!
But you gotta do something. 'Cause if you don't, it is only going to get worse.
Good luck to you.

How long have you been feeling this way? Was there a time when you felt different? Can you get back to that time? When people are troubled it is almost impossible to think things can change, but they can. You have the power to do anything you want. The best way to start feeling better about yourself it to start doing something for somebody else. Can you volunteer someplace? Google the name of your city and the word "volunteer" or "volunteer opportunities" and you will find tons of things to do.
